What MPV would you recommend?

I like to sit up high in a new car following hip problems, comfort and fuel consumption is important to me. Does one get better fuel consumption from manual versus automatic?

Yes, a manual will almost always be more fuel efficient than an automatic because it gives you a better chance of driving it economically. Small: a Hyundai ix20, KIA Venga or Ford B-MAX. Bigger, a Ford C-MAX or (my favourite) a Volkswagen Golf SV 1.4 TSI 150 ACT manual.
