How can I unlock my hybrid if the 12v battery is flat?

The battery on our Lexus 450h died to the point where entry into the vehicle was not possible, even using the key in the door lock. The car was no better than a brick.

Are there any SUV models which have mechanical door mechanisms which still operate when the 12v is totally dead?

An SUV is preferred as I struggle with arthritis.

The majority of modern vehicles rely on the 12v battery to operate the door locks, although a mechanical override is available using the physical key. However, as these are not used frequently they can be difficult to operate. Some models, including your Lexus, have a hidden level underneath the door handle which you have to use in conjunction with the physical key to unlock the doors in the event of 12v battery failure.

If you need to leave your car parked up for a significant amount of time we would advise using a trickle charger to maintain 12v battery health, such as the range of devices from CTEK - kit.honestjohn.co.uk/reviews/review-ctek-nxt-5/
