Pension - PDU1 - Collos25

I have to provide from the UK a form PDU1 part of the transfer of my pension rights to Germany has anyone ever heard of this form and if so where I am likely to get it.

I need the same form - having searched both the DWP and R&C sites, I could not find any reference to PDU1.

I'd be grateful for any help.

I have talked again to the German authorities and from what I can learn you have to go your local tax office you last used in the UK and ask for it, they showed me one that had come for a client and it had taken 3 months so form does exist,I am in the UK in January so will give it a try .
