Rough running Volvo V70 - what's wrong?

I have a 2008 Volvo V70 D5 Auto. At approximately 1600rpm the car stutters and feels sluggish. In park, at the same rpm, there is a roughness / imbalanced feel to the engine with the car vibrating slightly. The dealer has done a software update and replaced a driveshaft under warranty, but the problem persists. The fuel economy is also poor, with just 34mpg. Any ideas what’s wrong?

The starting problem is because not all glowplugs are lighting up. 1600rpm is about where the turbo should kick in so if that is on its way out that would explain it. 34mpg is not bad for a big, heavy automatic car.
