Why is my named driver's 'incident' affecting my premium?

I arranged insurance with LV for my new car. I asked for two named drivers to be added. Was asked if they had any claims or convictions, and the answer was no.

However, there was an 'incident' sustained by one of the named drivers last year. This was not her fault, the car was repaired and she did not lose her NCB. LV discovered this incident and loaded the premium.

I am not pleased on two counts. One, the imputation that I withheld information i.e. I'm a liar. Two, the premium is loaded for an incident that wasn't her fault. Is this normal behaviour on the part of insurance companies?
