They do break. Sometimes they make a banging noise as they rotate out-of-balance, sometimes a screech as the inner part turns and pulls the outer around more slowly just on the friction of the metal, but then you'll notice the power steering getting heavy and the headlights going dim because the alternator is turning slow. These rubber-loaded pulleys are a 'harmonic vibration damper'.
