Will grotty suspension bushes cause an MOT failure in themselves, or does there have to be excess free play?

What do you mean by grotty?? If they are rubber and perished then possibly so. If they've had oil on them and the rubber is 'melting' then possibly so. If the rubber is parting company with bonded metal bits then probably so.

Metal-rubber separation is usual reason for failure.
