What has caused my wife's two-year-old Volvo C30 to suffer a broken dual mass flywheel?

My wife's Volvo C30 2.0D has just suffered a flywheel failure at 2.5 years old, 21,000 miles. Volvo are replacing it under warranty but she is concerned about the cause, and whether there likely to be any future implications.

A dual mass flywheel is intended to reduce vibration in the powertrain of a 4-cylinder diesel and absorb some of the torque reaction in changing gear, particularly the upshifts between first and second and then second and third. Some are better than others, but obviously the less reaction in those gearchanges the longer the DMF will last.
