Volvo XC60 puncture - do I have to replace all 4 tyres?

My question relates to my Volvo XC60 AWD but I believe relates to most 4x4 vehicles. If I get an unrepairable puncture on one tyre, do I need to replace all four at the same time? I have read about transfer box problems but surely replacing three good tyres because of one puncture is not right? If so, why does my car come with a space saver spare wheel, surely using this would also cause transfer box problems?

The space saver is a temporary spare. The wear to the system occurs from constantly running with a disparity between tyres. Not much you can do if a tyre destroys itself half way through the life of a set (though, thankfully, this is rare). But it does make sense to swap tyres front to back same side about half way through their life (say about 10,000 miles).
