SWMBO has a new Avensis Verso, and has reversed into a small bollard already. Apart from the repair cost this is good as it allows me to have a minor prang in the future. As for solutions I am aware of reversing sensors and the Fresnel stick-on-lens. She would prefer a mirror. We have occasionally seen apparently good quality mirrors attached to the rear roof and even one extending from the factory rails to cover the back of the car. The dealers and accessory shops around here claim no knowledge of such things.
Does anyone know where we can get a decent tailgate mirror for a MPV? Do they interfere much with the tailgate lifting up?

The Jap imports seem to have them as standard. Maybe you could get one from one the import parts suppliers.
Depending on how deep your pockets are, you could try a reversing camera with LCD screen - I saw some for about £200.

I do believe toyota put a mirror on the rear of their "lucinda" or is the "previa" (not sure, maybe both) as these are a family vehicle/minibus/people carrier.
Maybe you could get a mirror from "Toyota" and fix on yourself.
