Why is my Mercedes-Benz Vito van smoking heavily from the exhaust?

My 2001 Mercedes-Benz Vito 2.1-litre diesel is suffering with very bad smoke from the exhaust. The engine seems to be misfiring with a possible chuffing sound from one or more injectors. It's suffered injector problems in past. Injector three has been changed and others re-seated with copper washers. It has suffered with the usual loss of fuel from leaks around the injectors. I am struggling with an explanation as to why there's so much smoke from exhaust. Anyone have any ideas?

It's an injector problem. Get it to a diesel injection specialist. If it looks like a huge bill, get rid of the vehicle.
